PHYSICALS
Winter Physicals
As we near the official start date for Winter Sports, please be aware that all athletes MUST HAVE COMPLETED AND PASSED A PHYSICAL EXAMINATION BEFORE THEY ARE PERMITTED TO BEGIN PRACTICE. Physical Forms are available on this web site by clicking on "links/forms" button, or in the main office. The PIAA Physical forms MUST BE FILLED OUT COMPLETELY (yes, all 5 pages) before an athlete can begin official practices.
If your family doctor can not schedule you in time, Med Express in Uniontown offers "walk-in" service and will charge $20 for a physical.
If a student-athlete played a sport in the Fall, they may NOT need another physical, but must have the parents sign the re-certification portion of the document.
